The Film Evaluation Form below is designed to be taken anonymously by teens after viewing the movie. It is important for us to be able to evaluate the impact of the film as an educational tool so your participation is requested. Simply print off the form, make copies and then have students fill it out after viewing the film. Collect the finished forms and mail them to: The Acadia Hospital, attention Alan Comeau, PO Box 422, Bangor, Maine 04402-0422. Thank you.

If you would like to know the aggregate results please contact Alan Comeau at: acomeau@emh.org.
